The Tennis Registration fee for 2017 is $220 plus $20 FTA membership fee, if not already a member. Scholarships are available for financial need. Please bring fee with completed forms to coach by July 26th, or your first practice. Forms will also be available at the courts to fill out there. Registration is Required.
Download, complete, save and return the registration form. You can email (firstname.lastname@example.org) or mail the completed form (FTA, PO BOX 73993, Fairbanks, AK 99707) or turn it into the coaches at the courts. You can make payment online below or enclose a check with your registration form.
The following forms are required. Either download, complete and return the forms, or bring them to the preseason event on Tuesday, July 25th:

- TAD – Tobacco, Alcohol and Drug-Free Policy
REMEMBER ALSO BRING Tennis Shoes and Water. Shorts with pockets are advisable. Bring your racquet if you have one. Loaner racquets are available.
URGENT You must have a completed physical before you can participate at the first practice. No practices will be counted unless the physical and parental consent forms are completed. Ten practices are required to participate in matches. Local health care providers also have physical exam forms in their offices. Physicals are parents’ responsibility and are good for one year. There are no in-school physicals available. Coaches will also have parental consent forms and registration forms that can be completed at the first practice.
